Sim switches to default upon exiting CAS

I'm not quite sure how to explain this, so please bear with me.
I'm trying to add a saved Sim to my world through CAS (while in Edit Town). But I have a problem.
CAS loads, Sim A shows up. I replace Sim A with Sim B (through the "Saved Sims" menu). I click the checkmark to finalize everything in CAS, but the portrait right before exiting CAS shows Sim A instead. I go back to CAS, and it shows Sim B with the body of Sim A. Only Everyday clothing is affected. I tried to change it but it doesn't work.
I thought maybe it was just a CAS problem, so I moved the Sim into a house, but it still shows Sim A. So I went to different save file and tried. Same issue.
I'm not quite sure what happened. CAS was working fine not too long ago. I even cleared cache before I loaded my game and cleaned the saves (which I always do before loading, anyways).

I do this very often and it should work directly, exchanging the sim in CAS and it should look the same in-game.
Maybe check the outfit, if the sim has two everyday now?

Or reset the sim, maybe it takes longer to update in your game.

Resetting and checking for duplicate outfits did not solve anything, unfortunately

One thing I noticed, though, was that not only the Everyday clothing was inherited — even the hair color, eye colors, and face details too! This is incredibly odd...

This is just a wild guess here as I have no idea of the environment in which your game is currently running. But might you have LD's Smoothness Patch 2.x with MC file in play? Or some other heavy-handed CAS-related mod that got introduced to your game relatively recently?

I do have LD's Smooth Patch + the MC file that comes with it. However, I've had it for a while now and I've never had issues with it until now I suppose. Once I get the chance, I'll check if it's causing the issue

There are known incompatibilities with MC integration

You can simply remove all additional packages which go to mod folder.
Delete Cache files and I am sure your issue will be gone.

Okay, it turns out I never had the MC file that came with 2.1

But I decided to remove 2.1 anyways, and I downgraded to 1.0. That seemed to work and CAS is behaving normally. Thank you C-Dark and igazor for the help
