I have finally decided to play some sims who want to run a resort. What has intrigued me, is that even though I am making a profit, so there must be some guests, the only sims I see at the resort when my sims visit are the sims who work there.
I thought it would be more like the base camps from WA where there would be random other sims who were staying there.

I've worked the resorts a fair bit and I do remember occasionally seeing a couple of sims visiting the resort and checking in, including sims who actually live in the town. That was with a custom resort I was building from scratch, no idea if that makes a difference or not. Sometimes other sims arrive instantly and other times it takes a few days. It does seem to vary though, since sometimes I get a handful of people around and other times I don't.
One thing I do remember noticing was how there appears to be a higher chance of sims visiting your own resort if there aren't many/any others in the town. Less competition for a sims attention, I assume. Are you playing in Isla Paradiso or have you created/placed a resort in another town?

I am playing Bayoux Beach, which is a small island. And because it is small, I had to create a custom resort that was small enough not to take up the whole world. Also, I find large worlds and large lots make my game lag. But I have only just started, so hopefully it will work as you say and the sims will arrive in a few days.

I hope it does for you. It is lovely to see some of the local residents milling about. It does definitely happen. I remember creating a custom resort from the bottom up in Isla Paradiso for my Brothers Green game (must get back to that!) and Honey Hamada came and checked in while on of my sims worked at the reception desk (he took a shine to her, took her out on a date and she went into labour on the date and he got upset because he didn't know she was married. Neither did I at the time. I sat there stunned for a few minutes). I believe a few of the other residents came over to be nosey after the resort opened, then it went quite but the money still came in, along with the reviews. So they're not always there but they do turn up from time to time.

You'll get two types of guests: resident sims and simulated sims (I've never seen service sims, homeless sims, or tourists from other worlds, but then I actually have tourists disabled with Register). Occasionally the game should push resident sims to check in and stay for a few days. But mostly the resort capacity will be filled with sims who don't actually exist. I think these 'simulated' sims are the ones leaving the reviews since you'll get reviews everyday even if no visible sims are staying there. Resort capacity is tied to the number of beds on the lot and (most importantly) to the resort rabbit hole. The eco-tower has the highest capacity when fully upgraded. You can add multiple resort towers to the lot to increase capacity if you have the space. If not, resort tower rabbit hole rugs are available - I think on simsasylum - which will accomplish the same thing (and can be upgraded twice like regular resort tower rabbit holes).
You can see the total resort capacity, and how much of it is used, in (IIRC) the resort finances screen. I've had a resort with a capacity well over 200 sims which was always fully booked with a 5 star resort. That was significantly more than the number of residents sims who actually lived in the world - and MC didn't show any increase in homeless sims or other types of sims to account for all those guests. This is why I've concluded that these invisible resort guests don't actually exist. Most of your resort profit is going to be from these 'simulated' sims. The residents who occasionally stay at the resort are primarily for show (but I'm sure they count towards profit too).

My Mum's had tourists stay at a resort she made to look like a guesthouse. They sat around in the lounge for a few hours, went off to explore the world, then returned to the resort. I'm not sure how long they stay for but they did seem to settle themselves there quite happily.

me too I'm a big, a huge fan of Isla Paradiso world and with that, of using resorts, so I've tested a lot with that
indeed, me too I can confirm that some other residents and/or tourists will occasionally come and visit your resort, while using the facilities, the food, the swimming pool, the gym, etc.
what's funny to me, to say the least, is that if I switch - using the Master Controler -> Make Active - to one of those residents, other sims from the town, (not tourists, as this is not possible for tourists), however, if and when I switch to one that can be played, (mainly other town residents) those are in fact not checked-in at the resort, as it tells you clearly "you'll need to check in to use the resort facilities"... hehe...
this is soooo, so funny to me, I can't even tell how much it makes me laugh... if I play the resort owner, roaming around and checking the resort, I see other sims from the town in the food area, in the swimming pool, in the hot-tub and such, having fun... if I switch right away, to another sim, that say, just finished eating, and I try to make them swim, I get the message they need to check in at the front desk to use the resort's facilities... so, makes me think, they've been enjoying until I took over them for free the resort's facilities, now that I control them, now I need to check in at the front desk... hehe... this is so funny to me...

I love the mods for TS3, makes the game so much better - a big "Thank You" for the creators for making the mods and sharing those with us all and for the admin team maintaining this site

That's unexpected! I've seen those sims use the VIP rooms and occasionally turn in at the resort towers too. I've played with owning all the IP resorts.
My observation about the presence of other Sims has been that just like with other public lots, when I have at least one active Sim on the lot, then there are other Sims present. If I don't, then there are few to none Sims present. Often the check-in desk, bars, and food stands won't even have Sims attending them until a while after one of my active Sims arrives.
