FYI, the older wiki links are "down" because Wikispaces itself went out of business several years ago. That's part of the reason that we are all here instead. We were going to transition eventually anyway, but the 2018 shutdown just sped things up a bit. Update, tuner does not appear to be working and isn’t showing in the mod scripts found on startup despite dreamer and saver being installed

I take it that you unzipped the download before placing its contents into Mods\Packages. Did you clear (delete) the scriptCache file after doing so? This needs to be done whenever the contents of Mods\Packages changes in any way and the cache will regenerate on the next startup of the game.
Tuning files like this one and standalone Tuning Mods are not script code and do not show on the popup list at startup. That doesn't mean they aren't loading.
I can test out the tuning file myself when I get a chance, but it worked the last time I tried it. Chances are that someone else reading this is already using it or will have time to get to it before I can though, so hopefully they will pop in with their comments.

I've had dreams disappear on a completely fresh install, in spite of having the NoTimeout tuning installed. I'm not sure if they actually timed out though, or if something somehow culled them, because I had three unpromised dreams all disappear at the same time, even though they'd all initially appeared at different times.
When additional dreams appear, do they overwrite the existing ones? I had two new dreams appear at around about the same time as the three original ones disappeared, but I'm not sure if it's linked, or just a coincidence. The mod FAQ suggests that extra dreams simply won't be visible until there's room for them, but even after completing some dreams, the original three are still gone.
Any ideas?

I added Dreamer and the no timeout tuning file a couple years ago, and my experience has been that while unpromised dreams usually don't disappear, it does sometimes happen when there are more than three competing for space. I'm not sure if the problem is having more than three unpromised dreams when I save/quit/reload, or if it's more random. The most recent one I can think of was the wish to grow a great or outstanding apple tree, I don't remember which, and it didn't survive the travel transition plus a handful of days in Egypt. The sim in question rolled lots of destination-related wishes over that time, and there were often more than three unpromised ones present.
I don't think I've ever seen an unpromised dream disappear when it wasn't achieved and there weren't more than three competing for space. And to be clear, I'm talking only about unpromised wishes; the number of promised ones seems to be irrelevant here.
It's much more common for me to only see three dreams at a time but to have the others hiding and show up again once I clear out the ones I can access. It takes switching from one sim to another, or waiting some time if I only have one sim in the household.
What I've always read is that having more than three unpromised dreams is unstable in the way that many Sims 3 features aren't completely stable, so while it works most of the time, there's no way to make the feature totally reliable.
On the subject of unpromised dreams, I've also noticed that while sims will receive credit for most of them, the ones surrounding teaching toddler skills don't work unless they're promised. I've seen it repeatedly, enough to say for sure there's a pattern. But I just leave space for those among the promised wishes.

Well, I tried again on a fresh new save, and I definitely saw my sim's wishes time out with no contention. I had four promised wishes, and three unpromises wishes, and the first two unpromised wishes (that had originally appeared at the same time) timed out, and did the usual animation as they vanished. A while later, the single remaining unpromised wish also timed out. This time there were no excess wishes that could explain it.

It sounds like you don't have the extra tuning package, or it's not working. Delete your existing version, if you have one, and download a fresh copy, then delete scriptCache.package and test.
