I can confirm that it happens even without a single mod, and that it doesnt happen every single time, but way too often. The only thing I had was the ddraw.dll file that comes with Smooth Patch and some things from Lazy Duchess' random sim fixes, both in the Game/Bin folder, not sure if those would affect anything. I completely removed my mods folder (moved it out of the sims 3 folder) and the only things that I left in my game/bin folder were the "ddraw.dll" file and everything from the Lazy Duchess shader fix (I was wrong about it being from random sim fixes, my bad). The issue is NOT that im getting errors, the issue IS that the UI collapses off to the side incredibly often upon deleting a pet and makes it so that I can not save the household. The error only gets caught when I click the checkmark to save the household or click on another sim portrait while the UI is missing. I do not need to test it with any NRaas mods because the issue is incredibly visible regardless of whether or not I get a script error file in my sims 3 folder. Apologies if that sounds harsh, it is not meant to be, I am just trying to explain as clearly as possible what is happening. I cant imagine that shader fixes would affect the UI of create a pet, and I do not know how to get the old version of ddraw.dll back (it looks important so I didnt remove it, despite coming from Smooth Patch)

If you are willing to pursue the theory that the Smoothness Patch or elements of it are the culprit or at lest part of the problem here as our developer says, I would suggest using Steam's Repair Game function to restore the game's program folder back to factory standard. Yes that is correct. I did not know I could do that, thank you for telling me. Oddly it seems to be less likely with repaired files, but it DOES still happen. I also discovered that the UI seems to only disappear when you delete the pet whos portrait is next to the sim. I guess the issue is it gets confused sometimes because it tries switching to the sim (on the left) but switches to the next pet (on the right) instead. The UI closes off to the side twice and then either reappears or just stays gone. When you delete a pet that is only next to other pets, the UI does not collapse at all, and therefore there is no issue. It also seems that Create a Household from Edit Town mode is more broken, because while I did have the UI disappear in the Create a Household from the start of a save, I was able to switch to a different pet and the UI reappeared, and I was also able to accept the household even with the UI gone. Yes, but it doesnt happen every time so it is more easily replicable if you put quite a few pets and then delete them one by one until you run into the issue. I think there need to be at least two pets in order for the issue to occur, actually, since its while automatically switching to the next pet that it disappears. You may need to try that a few times, or it may just immediately happen. I put my mods back in because I was unable to record the video without the fps limiting aspect of smooth patch. Encoding overload in OBS or something. Like I said though, the problem occurs with or without it, but with it it happens more frequently. Frequency makes demonstrating the issue more easy anyways
Apparently I was wrong about the distinction between a new game and editing town, because as you can see, I was completely unable to do anything except move the dog around and that was a brand new save. Hopefully you can see the differences in what the UI does. This time it just completely disappeared, but once in a while the sim UI shows up for a split second in place of the pet UI and then either switches back to the pet UI or disappears

The wording in the UI is not vanilla, the EA CAS does not say "Assigned Gender", this seems to imply you have some kind of UI mod (or your own edit to the game files?) still in play. Unless Smooth Patch does that and I've never noticed? [Edit: May have misread and thought you only replaced smooth patch to make the recording, not every mod. Disregard if so.]
Also, are you testing this in new saves each time or the same pre-existing save? Just pulling all the mods and smooth patch out mid-game might not actually remove a bug one of them was causing. It's quite possible for data to be written into a save itself, as well as the caches.

Testing new saves each time. And yes, I do have a "gender neutral" mod that simply changes the text strings afaik (the "assigned gender" text is actually kind of annoying. i wish it said sex instead but thats irrelevant to the situation). I put all of my mods back in for the recording because the issue happened even with no mods so there was no point in recording it with a vanilla game. Tangentially, I think I even remember it happening once before I downloaded any mods. I do not know what to look for in order to fix it, or if it is even possible to fix it, but if I could I would make some sort of mod that eliminated the issue. Maybe I could somehow prevent the UI from collapsing altogether when switching between pets and sims? I think the issue is related to the framerate, because this time I set the framerate cap to 30 instead of 60 and the issue now seems to happen every time rather than almost every time.
