I have deleted the old MC 134 version, cleared my cache and installed the MC 135 version.
I don't have AwesomeMod installed (removed it few days ago).
When I load my save, the error message says "Careers recorded: 1650" and I am no longer able to assign afterschool activities even the EA ones. For both children & teens, it says "no choices".
I tried 3 times so far, same results each time.

Interesting. I wasn't getting any error messages so I thought careers seemed fine with the changes. Admittedly I only tested with twallans afterschool activities installed though. Couple things:
You have an afterschool activity installed that requires the violin from the store (Music Lessons) but don't appear to have it installed. Also is the "Hospital Mod - Sept 2012 - for supernatural" the hospital overhaul mod? If so, that one hasn't been updated and causes issues.

Great points My error log files are from about 5 different saves.
- One save, I had installed Missy's Music ASA, interestingly I had installed the "No Store" version so I'll look into why it's still considering I need the violin instrument...
- Another save, I had the Hospital mod installed but I read a thread where you were saying it was causing issues with mods loading *after* it so I uninstalled it stat.
- Final save, I had none of the above installed and the "no choices" issue still happened.

I was puzzled by the sudden appearance of an error log and forgot to follow up on this. Are you trying on multiple different Sims? Just tested myself and it's working fine! I added Bella Bachelor to a custom activity via Master Controller and it's worked! Thanks, Chain!

Ah ok! Chain_Reaction, you were correct about the "Music Lessons", there is no "no store" version for it *walk of shame*. I confused it with Missy's 10 Extra Afterschool Activities, that does.
So I reinstalled all of Missy's custom afterschool activities for the purpose of today's test save.
My report:

So I reinstalled all of Missy's custom afterschool activities for the purpose of today's test save.
My report:

- I fired up a fresh Isla Paradisio save. I made sure none of the problematic mods you had pointed out were installed.
- I was able to load the afterschool activities by EA on the first load so there was no "no choices" popup issues on the few target sims I had clicked on, this time.
- But I still couldn't see Twallan or Missy's custom afterschool activities.
- Child-aged female sim (Emily Hamada) who was 4 days out of 7 in that age phase and who was not enrolled in any activity generated the "no choices" popup again. I used "MC > Intermediate > Absolute Age" to reset her age to "0" and it freakin' loaded all the custom activities in the menu and I was able to assign her one of Missy's activities! *happy dance*
- The same issue as above happened with a male child (Omar Collins) aged 6 days out of 7 in that age phase. Resetting his age to "0" days out of 7 fixed it and I was able to successfully assign him one of Missy's activities.
- One female teen (Ashley Alto) who was only 3 days out of 14 in that age phase, couldn't receive afterschool activities at all. She had no job. I assigned her to a school: still no "activities" in her MC menu. I reset her age to 0: still no "activities" in her MC menu.
- Same for male teen (Sean Vidal) who was 9 days out of 14, enrolled in school and part-time mausoleum employee level 2. He had no "activities" menu option at all. I MC-resigned him from his job and MC-reset his age to 0. Still no "activities" in the menu.
- One male teenager (Joel Vidal) who was only 1 day out of 14, could receive afterschool activities but it showed the "no choices" popup. He was already enrolled in school. I reset his age to 0 and I still got the "no choices" popup.
- One female teenager (Chandell Scott) who was 13 days out of 14, had the custom activities showing up right away.

- Ashley Alto: there still was no "activities" menu showing up in MC for her. I guess she's a broken EA sim somehow?
- the Vidal Bros: the game reload seemed to do it for both of them. This time, the custom afterschool activities showed up just fine in their MC menu.
