I didn't run a test to double check, but I've been on 1.69 since I started playing over five years ago. I have never had this problem, presets always save as expected, both on patterns and on objects. And I use presets quite a lot, I even save recolours in collections sometimes and they work just fine.

Actually TS3 isn't buggy on Win 11 is just that Win 11 doesn't like disc installs due to the DRM they have.

I uninstall OneDrive as soon as possible because I don't use it and I just find it to be a nuisance. I just repaired the game, started it unmodded, created a new save file, saved some patterns and my userpresets.package file is not empty! It looks just like you described. The presets also show up in game now. Since I have a working userpresets file now I guess I'll try playing modded again and see what happens?

Okay, so the patterns that I saved while testing the unmodded game saved properly and survived a relaunch. However any new patterns I tried saving while playing with mods did not stay. Only the ones I saved unmodded prevailed. It seems to be a mod issue.

If you ever narrow down what the culprit is, it'd be good to know.

I'll try removing some mods I installed recently as this wasn't an issue before that, plus I'll revert some stuff I did with my CC just in case. Hopefully I won't have to do too much digging into my mods until I find the answer!

I figured it out. The culprit is "Move Objects Cheat with hotkey" by Eca on MTS. It's also the mod that caused the crashing during game exit. Looking at the comments on the mod's page shows that other people have also encountered the same problems as me while having it in their game. The mod creator seems to be aware of the issues, and they tried to fix the crashing issue, but they haven't been able to, it seems... So they're yet to do so. I guess I'll just have to remove it. At least there's no more crashing while quitting and my presets will save now! I think I should start reading the comments on a mod's page before downloading it...
