no, normally not while the game is running 
If you f.i. rename it or move it - the game cannot access it anymore,
I mean, you can try, but don't save, only quit, because your sims might wear it.

Is it okay to organize your cc while the game is running?
Generally, no. I don't think it would break anything long-term, but for that instance everything in the My Documents directory is loaded on startup and should usually give an 'in use' error if you tried to move or rename it. The game also won't read anything that was changed or added until it reboots. Files in use should be left alone.
