The initiator is the one who initiates the Try for Baby interaction. If you're controlling things, it's the sim whose actions you direct when you click on the other sim (the target). Woohooer will attempt to impregnate either or both of these sims, depending on which setting you choose. For example, if you pick "initiator then target," the mod will try to impregnate the initiator first, and if that fails (like, for example, if the initiator is too old or already pregnant), then the target. If you pick initiator only, then the target sim won't get pregnant from the Try for Baby interaction regardless of what happens with the initiator.
If your sims are getting frisky autonomously, you can tell who initiated by which sim is showing the Charisma progress bar when the interaction starts—that is, if the sim has learned Charisma but hasn't yet maxed it out.

Thank you so much puzzleaddict, i think what was getting me confused was my initiator must have already been pregnant so the target sim was the one who got pregnant. Makes since now....thank you for the explanation.
