Hello! I was looking for the closest thing to a pregnancy scan and something similar to this showed up (screenshot is not mine). I was just wondering why in my game I don't have the gender information and what do those percentages mean? In the last pregnancy it said 500% chance of quads but my sim only had a single child?

Everything is showing except the gender? My guess would be that it has not been determined or set by StoryProgression for that pregnancy yet, then. If the gender of the first born of a pregnancy is meant to be Random by mod or otherwise, and in-game tricks are not deployed to set or reveal it one way or the other, then I believe it should remain undetermined until the time of birth.

The chance of quads must be coming from Woohooer as it's the only mod (I know of) that even allows for those. Under EA standard, triplets is supposed to be the highest a multiple birth will go. As I recall the displayed percentage chance of quads has always been flaky, it's possible that 500% really means something more like 0.05%.
