Items being moved to family inventory
it has never happened to me before. I returned to one of the worlds I’m playing rotationally and realized that items from my sims’ personal inventories are all moved to my family inventory... Does anyone know what causes this..?

The game does that on its own. It's very annoying, I'll agree with that, but I don't think anyone has ever found a way to prevent it. I get this even without switching worlds and can see it when I use MC to periodically query inactive households around the world being played to see what might be in their personal and family inventories.

Some say MC reset does this, some say it's SP's inventory management... haven't been able to reproduce it to nail it down.

Actually, I did both... I did reset everything and also that option was turned on... hmmm.... that option has been turned on for a long time but that had not happened but I recently did reset everything for a different reason so I guess that might be why..?
Also, I tried to put those items back in my sims’ inventory but I couldn’t put that portfolio thing for achitecture sims back into my sims inventory. Is there any way to force that through nraas mods?

I think in my case, reset everything definitely causes this. It has never happened to me before until I use reset everything. After that first post which was the first time I used that feature, I used it again in a different world recently and it happened again to some of the sims. Their items were moved from their personal inventory to their family inventory.
I could just simply move some of the items back to their personal inventory but I can't move the portfolio tablet thing for the fashion career.back to the sim's inventory. I can take it out from the family inventory and put it on the ground but it is not movable in the live mode.
Does anyone know how to put this portfolio back in the sim's inventory..?

Okie I solved this issue too. I guess it can apply to any item that are supposed to be in personal inventory but are stuck in the family inventory or outside.
Use debug enabler and use "move into family inventory". Then click the sim you want to put the item in and use debug enabler on that sim. Then click "move from family inventory". Then you can put any item that are stuck in the family inventory into personal inventory.

I tested this inventory issue, and it is definitely caused by the "Reset Everything" command through MC.
[Hypothesis Debunked] I think I have a solution for the issue, though. I noticed that the Sims in households I had recently played were not losing their personal inventories into the family Inventory. I tested this and it appears that if this matters to you, "Reset Everything" has to be a planned event. The solution is to load the game fresh, look at the inventories of all the Sims in your current household, and then "Switch Households" (use the button in the UI, not the "Household Information" pop-up; my game crashed while using that one, although it could just be happenstance, I didn't test too many times because it's time consuming) and switch to all the households whose inventories you want to protect. Actively look at each Sim's inventory. I purposely skipped a few Sims and while their housemates didn't lose their inventories, those Sims did. Once you've done all that, it is time to "Reset Everything." You definitely want to save immediately after, and then you'll just want to quit the game and reload it because I immediately got "Error 12" after playing for a little bit.
I rotate every household in Sunset Valley, so the loading a fresh game and immediately reloading after resetting might not be necessary if you only want to protect a handful of households, but it's probably better to be safe than sorry. [/Hypothesis Debunked]
EDIT: Nevermind. Turns out it was somehow coincidence. I have no idea how some Sims in a household could be affected and not others (and be the ones I didn't check), but whole households I definitely checked had their inventories switched up.

I've discovered editing community lots where inventory items have been left throws them into the family inventory. DebugEnablers Set Lot Owner Null stops this behavior. But there's still probably the connection to reset everything as well.

I was wondering why I kept missing my sim doctors pagers! They always end up in the family inventory lol. I can definely confirm 'Reset Everything' causes personal inventory items to end up in the family inventories. I just use Debug Enabler to move them back.
