I'm currently running V268, but haven't noticed any relevant changes in the V269 test builds.
Currently I have an issue where if any Sim fights another Sim in a story, they're *both* considered to have won the fight: the victor receives a story that they won, and then the loser receives a story that they've won, and both the victor and loser will then check to go to the hospital.
Another problem I've noticed is that when a townie is electrocuted non-fatally, a bunch of inheritance stories are generated anyway.

Actually, come to think of it, I don't think I've seen anyone die as a result of the push death chance. I've had a few situations where inheritance stories popped up but the sim actually didn't die: I recall more than one instance where I had to pause and look at someone, thinking "I thought you died?"
I'm using an external tune as my default -- I wonder if it's just a matter of not properly loading from the external tune and thus defaulting to zero chance even though the in-game menu displays the (25) chance as it should. I notice that tax days also don't tend to load from tuning properly either -- the tuned day(s) applies to the active household but the untuned hard-code defaults sometimes apply to townies until the in-game tuning for tax days is manually changed (even if one day is just toggled on and toggled off to populate the "change").
